PROFILE: Established in 1999, the Connecticut Health Foundation (CT Health) is based in Hartford, Connecticut. It seeks “to improve the health of people in Connecticut.” CT Health is a conversion foundation, created when ConnectiCare transitioned from a nonprofit HMO to a for-profit company. In a state with great economic disparities, the foundation focuses on health policy and leadership and health equity in its program areas.
Grantmaking focuses on health coverage and access to primary care providers for regular care. CT Health supports safety net health care providers, Medicaid availability, and programs to raise the rates of insured citizens in Connecticut. Another focus is healthcare delivery system design, including efforts to foster an environment where underlying causes that contribute to diseases are addressed—not just symptoms.
CT Health looks for grantees that link care to community because factors that affect health exist in the overall environment and lifestyle. It supports community health workers that help Connecticut residents improve their lifestyles to save money in the healthcare system and improve their own health outcomes. In terms of advocacy and leadership, CT Health supports a Health Leadership Fellows program and aims to advance health equity.
CT Health funds nonprofits, state and municipal public agencies, public entities like health departments and social service programs, advocacy groups, and collaboratives. However, it does not provide funding for direct services, clinical research, capital projects, projects that don’t affect the people of Connecticut, or indirect costs for general operating support. Support has gone to Connecticut Oral Health Initiative, and Community Action Agency of New Haven. Grants typically range between $10,000 and $80,000.
